SUMMER PHYSICALS - ATTENTION FAMILIES OF 5TH AND 8TH GRADE STUDENTS! If your child is intending to play sports this coming school year, this is the time to schedule a physical. Call your health care provider to make a summer appointment. Click here to download the form your health care provider must complete.
Student athletes continuing to participate in the athletic program must be re-examined within two years of the previous physical examination on file. For more information on District Policy, see links to JLCA and JLCB below.
IMMUNIZATIONS - ATTENTION FAMILIES OF 5TH GRADE STUDENTS! Students entering sixth grade must have completed two doses of Varicella (chicken pox) vaccine, or have a history of disease as reported by parent or health care provider. ALSO, they must have a Tdap vaccine after they turn 11 years of age if it has been more than 5 years since their last tetanus-toxoid containing vaccine. See the table below, right side, under Other Information OR visit the DHHS website for more information on required vaccines.
Please contact your health care provider immediately for an appointment to have these vaccines administered, or to obtain a history of disease. Included with the letter being sent to all Incoming 6th grade families is your child's School Immunization Record. Please have this with you when you call your health care provider. The school health office requires written documentation of the above immunizations prior to the first day of school on September 7, 2010. If the above vaccines are not on the paper included with your letter, you will need to send us an updated Immunization Record.
INFLUENZA - The NH Department of Health and Human Services, NH Department of Education, US Center for Disease Control, US Department of Education, and US Homeland Security work closely to assist schools in preparing for health and emergency situations.
